Introduction and Background
Chartered society of Healthcare services is a healthcare institute that would be offering unique combination of premier healthcare services and community based social services to Southeastern England. It is planned by organization to create their facilities by the end of 2017.. Diabetes patients and patients diagnosed in early stage will be the consumers of the services. The primary founder of the organization is social worker having prior experience in operating a social service agency (Tulchinsk and Varavikova 2014).
The business proposal rationale is constructed upon the prevailing condition of UK for fighting with diabetes. Scope of business plan is to describe business care for implementing provincial diabetes initiatives for all the individuals with diabetes. This involves adults with Type one and Type two diabetes and children. Diabetes business proposal is chosen for focusing on elements of managing disease and enhancement of avoidable consequences and impediments. Every year, diabetes is diagnosed in approximately 19000 British Columbians (Gov.uk 2017). Diabetes has devastating consequences for health and is considered life threatening and progressive as well. However, for improving the outcome of people and preventing diabetes complications, there can be a great deal that can be done. The rationale of business is to outline resources and requirement for diabetes initiative to design the system. It would support managing diabetes effectively by providing comprehensive, excellent and integrated care. The proportion of people with long-term health conditions is rising in UK with as many as 80000 people living with diabetes (Gov.uk 2017). There has been exponential increase in number of diabetes death and the total number of deaths to 12000 at annual basis. There are major gaps in traditional medical system, it has remain unaddressed, and the development of this particular business plan will help in addressing existence of such gaps.
The proposed social enterprise model will help in addressing the gaps and pain points existing in current diabetes ecosystem. Fundamental objectives and rationale of business proposal is satisfied through an exhaustive resource interface. There would be technological enhancement relating to the marketing techniques in the proposed social enterprise. It would lead to generation of desired revenue and investment for the sustainable cause itself.
There has been a fast rise in Type 2 diabetes and this has increased the chances of people with obesity. It is expected that there would be new diagnoses of diabetes patients by year 2020. As the population ages, the number of people living with diabetes is expected to increase to 4.6 million by year 2030 as compared to current population of 2.7 million (Adgate et al. 2014). It is required to deliver faster and earlier diagnosis of diabetes and improving the experience and outcome of patients. There is a need to drive down the waiting times, developing a diabetes-controlling program and increasing diagnostic capacity. More than 2000 people with learning disabilities can be provided with better chance of life in the community (Bongar and Sullivan 2013).

Initial team of the organization would consist of clinical director, one administrative director, and four employees. It would also involve contracting agency that would help in fulfilling the need of occupational and physical therapists. There would be administrative assistant and a licensed social worker who would help in providing administrative direction. As there will be growth in demand of services offered, Chartered society of healthcare services would be hiring the job of service providers. Development of a successful social health enterprise requires leadership that is committed to engaging staffs in decision making and enabling them to address the challenges. Management structure of the organization would have need of staff engagement that would be a permanent feature of structure of management (Schaper et al. 2014). This would give the staff members with a sense of accountability that lacks in most of the social enterprise.
Chartered healthcare business would require necessary legal support to grow in this sector. There can be variation in the legal framework in the range of organizational model. Some of the legal structure that can be considered by organization are as follows:
- Intention of social enterprise is regulated for ensuring that the assets of organization are protected and they are fulfilling their social ambition.
- The charitable status and structure of group is established for tax reasons and where the intention of organization is to retain surplus. This usually happens when equity is not considered as source of finance (Schneider 2016).
Chartered Healthcare services can be established as a trust or an unincorporated association or company. If the organization is established as a trust, then would be governed by trust deeds and they get the tax advantage as they are established for social goods. Since the organization would be set up privately, they would ensure to look after the family members. Establishing the organization as trust would oversee the property held for community. This would reduce the cost on the public purse as the control of assets are with the community. Establishing the organization as unincorporated association would help in establishing the procedures quickly (Khoury and Ioannidis 2014).
The critical and expertise workforce of Chartered health service along with sound five-year capital plan and the flexibility of the services. It is done to meet the changes in demand and care is increasingly delivered by providing instant diagnostic services for diabetes patients. Some of the significant investment is made by the organization in facilities, equipment and informatics infrastructure and allowing expansion for flexible capacity. Service provided by Chartered Healthcare would be based on new care model for addressing the challenges faced in providing long-term healthcare services (Blank 2013). This would involve driving productivity and improving the experience of care with people with diabetes.
Emergence of revenue generating activities for social enterprise such as Healthcare has resulted in creation of new operating model. Under such model, market values and characteristics, business principles co-exist and they work by forming alliance with the traditional public sector values such as serving the public interest and responsiveness to community. There are two elements involves in operating model of Chartered Healthcare services. It involves a resource strategy that would define the terms and timing of resources to be acquired. Operating model would also involve external partnership and internal structure of organization that is considered for creating the impact on organization. Such model act as channel that enables the social enterprise to convert input into outcome (Betancourt et al. 2016). It would also lead to generation of economic and social value.

Chartered Healthcare services seeks to develop and have continue improvement by delivering quality services year after year. There are some concern about sustainability in terms of external environment and available support for developing competencies. It would be establishing a clinical outcome unit that would be working with allied health professionals, doctors and nurses. System of the organization would be extended and it would incorporate allied and nursing healthcare professionals. Sustainability in the health services would be imbibed by monitoring, analyzing and acting upon the all the aspects of diabetes patients. There would be immediate improvement in the patient care and management and this would directly lead to effective improvement in the outcome of individual patients (Furrow et al. 2016).
Competitors
Chartered healthcare services needs to consider political, regulatory and technological factors when addressing the challenges and designing the sustainable strategy. It would be required by the organization to establish a business orientation so that there is financial sustainability and growth. In the wake of changing political environment, it is a matter of concern that whether the emerging enterprise would be a subject to bureaucratic and political interference (McKeever 2016).
Conclusion
It can be concluded from the above analysis that Chartered Healthcare would be able to achieve competitive advantage in imparting the social services and at the same time generating profits for healthcare. Organization is committed to provide quality services and care to diabetes patients. Services is intended to improve the overall health of people suffering from diabetes. It can be recognized from the sale forecast that the organization would experience high rate of growth in the first two year of operation. However, it is certainly possible that in the third year of operation, the possibility of declining profits is apparent. This is forecasted taken into consideration various macro-economic factors and some factors attributed internally to the organization. The capital structure of organization would consist of both equity and debt. From the projection of cash flow, it can be easily depicted that there exists gap between the actual and forecasted cash flow.
