This letter is a final and written warning for unsatisfactory job performance. As discussed earlier, we had some concerns regarding management of your performances that had a record of duties at the division. Also you did not provided any acceptable explanation as why you had not met with the desired tasks. You failed to fulfill your duties with integrity and sincerity and all your disciplinary actions may result in the termination of your employment.
Further diligence is needed and performing your future duties. If this happens again, additional disciplinary action would be taken on a serious note. Also the termination of employment will be inevitable.
A copy of the same will be kept in your file at the office.
