Delivery of Palliative care through a multidisciplinary approach
An effective care plan includes the focus towards both the patient behavioral needs as well as their conditional needs to ensure the delivery of palliative care. In the case of Mr Jagger, it can be reflected that there are certain key areas which are to be focused upon including the care level or quality of care required by the patient, the issues and challenges being faced by the patient as well as his family members at both the physical as well as the psychological level.

Such is important to be considered, as Mr. Jagger had a medical history of bowel cancer which was treated 12 years ago, but in the recent diagnosis he has again been diagnosed with bowel cancer and his condition can be identified to be deteriorating. Therefore, it is important for the professionals involved in the multidisciplinary team to have expertise towards bowel cancer and can contribute towards providing quality and advanced care as per Mr. Jagger’s present health situation. Borgstrom et. al (2021) supports that in the present approach to healthcare multidisciplinary teams ensure that the group of professionals having effective industry understanding can be aligned with that patient’s health needs.
Conclusion:
It can be reflected that having a focus towards a multidisciplinary approach ensures that the challenges involved in the process of care delivery can be evaluated and further with the help of an experienced professional such can be eliminated. In the healthcare process, it is important that the professionals should be able to provide the required support to both the patient as well as to the family members, therefore, in such a case an approach to multidisciplinary and comprehensive care act as a key approach to be considered.
Strategies to support families and carers
The case of Mr. Jagger reflects that in the delivery of quality palliative care it is important to ensure that the families of the patient as well as the individuals providing the care to the patient are also being provided with support. The strategy to respite care as well as emotional care can be adopted to provide healthcare support to carer.
Explanation
A key approach or support strategy that can be adopted in the case of Mr. Jagger is proving respite care to the wife of the patient (Swartz & Collins, 2019). From the case, it can be identified that the wife of Mr. Jagger is worried about the deteriorating health status of her husband, and such is expected to have a major impact on her mental health as well as her workload in managing both Mr. Jagger’s health needs as well as the daily activities. Liao et. al, (2022) defines that adopting respite care will allow his wife to take a short break from her responsibilities and can focus on her self-care and reduce the stress or the impact on her mental health due to her deteriorating condition of Mr. Jagger.
Leocadie et. al, (2018) have reflected that adopting the approach to respite care can allow Mrs. Jagger to take a break and ensure her health status, but despite such effectiveness there are certain key issues and challenges associated with the strategy which is to be considered. Such includes the lack of proper planning and execution of the care plan. As Mrs. Jagger can be provided with a break from her responsibility, but Holliday et. al, (2022) has defined that during the time planning should be done that can engage her in other leisure activities which can divert her attention from Mr. Jagger’s health condition, so that her stress can be relived. But the lack of effective planning towards such areas results in affecting the effectiveness of the respite care for carers.
Bjørge et. al, (2019) has further defined that an approach be adapted to provide the required support to Mrs. Jagger as a carer includes providing emotional and virtual care support. Such may include assigning a psychologist or a care nurse who can interact with Mrs. Jagger in specific time periods so that the emotional issues or any physical challenges being faced by Mrs. Jagger in providing care to the patient can be identified and further can be eliminated. According to the study being conducted by Jess. et al, (2019) such care can be provided through the use of technological advancements or the use of digitalization in the industry. However, Kamal et. al (2020) has defined that in the delivery of such emotional and virtual support to the carer, it can be reflected that there is a need for having a nurse that has abilities to influence the carer so that the challenges or issues being faced by them can be identified and accordingly the strategies can be formed (Moss, Kurzawa, Daly, & Prince-Paul, 2019).

The defined strategies can be utilized considering both the positives as well as the negatives of the strategy and based on the situational or the individual needs, the required strategy for carer support can be adopted.
Integrated palliative care for Mr. and Mrs. Jagger
The nurses in the healthcare sector are further required to focus towards the development of key strategies and approaches that can ensure the implementation of integrated palliative care practices for patient health improvement.

Steigleder, Kollmar, & Ostagathe, (2019) define that an effective approach to delivering palliative care in the nursing sector is to ensure that overall monitoring and control of the nursing process so that the key gaps and issues in the care plan can be identified and eliminated. In the case of Mr. Jagger, the way that can be adopted to ensure the delivery of palliative care includes the focus towards a holistic approach. Sapeta et. al (2021) has further defined that in healthcare one-size-fit for all approach does not exist or cannot be validated, therefore it is important for the professional to adopt situational approaches and practices which can guide upon patient’s specific needs and interests in the industry.
Metaxa et. al, (2021) has defined that in the approach to palliative care, collaborative care practices are to be adopted that can ensure aligning the family members of Mr. Jagger including his wife within the process so that they can be informed and be educated about the healthcare processes and approaches and further influence them to contribute towards the improvement of patient’s health condition. Further, in the case of Mr. Jagger, the approach to integrative palliative care can include a focus on advanced care planning (ACP). Khan et. a, (2017) have conducted an industry study which reflects that advanced and integrated care planning ensures that the nurses adopt approaches which can strengthen the care plan, such includes adopting an advanced approach to communication among the professionals as well as the family members of the patients, identifying substitutes or alternative planning processes for the delivery of healthcare and providing support to both the patient as well as the carers involved (Brazil, 2017).
Mr. Jaggers has been facing the issue of bowel cancer for the last 12 years, therefore there is a need for the nurse to ensure evaluation of Mr. Jagger’s medical records over the period so that the key issues or the health challenges being faced by the patient over the period can be identified and further be eliminated. Such can further promote accountability as well as transparency among the patient as well as the healthcare services

Based on such an overall analysis it can be reflected that it is important to develop a conceptual understanding at both the professional as well as the patient level so that a collaborative and integrative palliative care approach can be adopted and further be implemented (Seow & Bainbridge, 2017).
