Health promotion can be defined as a behavioral social science that has been drawn from the biological, physiological, environmental, medical as well as physical sciences in order to promote health as well as to prevent diseases, premature death and disability with the help o education-driven voluntary change activities. Health promotion is highly crucial in a community since it has the potential to enhance the quality of life of individuals. Considering the fact that the chief objective of health promotion program is to prevention rather than cure, health promotion reduces the cost that individuals spend on medical treatment. Western NSW Local Heath District (LHD) has been suffering from several health related issues like high smoking an obesity rate in both adult males as well as females. In order to eradicate the issues by educating individuals and implementing prevention methods, a health care program will be conducted on the mentioned area. The targeted population comprises of adults residing in Bourke. The purpose of the report is to develop and effective action as well as evaluation plan for Western NSW Local Heath District (LHD). As a part of the action plan, a simple food and nutrition intervention project will be designed in order o address the high priority requirements of the community. The first step for conducting the plan will include a Mandate of action, followed by identification of key stakeholders & multi-disciplinary program team. Finally, a Program action pan and evaluation planning diagram will be developed.
