Part 1
The aim of the current study is to develop upon the personal development plan that is aimed to improve upon my skills as a professional nurse. Effective personal reflection will be used for the following purpose that will help to evaluate the experiences that I have gathered from the previous project experiences. For the professional development plan, health assessment and communication will be used along with that of health promotion and disease prevention programs. In the following part, most of the important matters related with that of the health care assessments needs to evaluate that can help in dealing with the issues of the existing health care policy. The health care behavior also needs to be one of the most important part and criteria of the health care assessment.
Population health assessment is considered to be highly essential in the given context of the nursing personal developmental plan (Laschingeret al., 2014). As a professional nurse, it is my duty to understand the healthcare scenario of a given location that will help to understand the frequency of the total number of patients in each area. For this, it is important to understand the important to understand the demography of a location. The quality of health care service that are provided to the people are also depends on the level of poverty. It is important to focus on the needs of individual group within the larger groups of population. This will help in the process of identifying particular healthcare challenges that are encountered to overcome the specific issues within the population. Hence, with the help of comprehensive focused assessment, it will be possible to identify the most vulnerable group within the society.
The nursing communication plan is believed to be one of the major parts within the nursing care assessment as it can help an individual to better connect with the community and understand their health issues (Wang and Liu 2015). As part of the social health care worker, it is my duty to understand the cultural background of the society that is related to my workplace. As detailed information has been collected from the culture of the community, it will be possible for me to understand the mode of communication that is used within the health care sectors. There are certain communication barriers that need to overcome by the community nurses. In most of the cases, the patients are not able to use effective forms of verbal or non-verbal forms of communication to provide detail information of the medical record and background.
Population Health Assessment
Beardwood and Kainer (2015), have used effective nursing communication tools that are considered to be essential in the matters of understanding the complex issues that are encountered by the patients. The aim in the given context is to gather accurate forms of information that are related to the current medical condition of the patient. The quality of the health care assessment is highly dependent on the accurate level of information that is obtained from the patient communication.
Regan et al., (2016), have mentioned about the ISBAR tool that is one of the important communication medium that are used by the professional nurses for effective modes information sharing. The primary elements of the communication tool are to identify the information depending upon the situation. Total background assessment will be done for each individual patient that will further help in the high accurate assessment of the patient. This will ultimately help in the purpose of providing healthcare recommendations to the individual nurse depending upon their clinical conditions. The risks that are involved in each of the cases can also be evaluated depending upon every clinical situation.
One of the important parts of the nursing Development Plan includes preparation for emergency situations. The first hand responses that are provided during the time of emergency are highly critical for the future treatment of the patient. I have gathered the experience of emergency care patient during the time of earthquakes. There was high level of pressure within the emergency care ward due to the increase number of patients. Effective decision making is highly essential in the Critical Care Unit, which will help the professional nurse to take immediate action. High level of awareness is also required among the nurse in order to act effectively during these critical periods.
Promotion of health care services and other essential element of nursing development programme. It is one of the important professional duties of every nurse to develop proper Healthcare promotion program (Eldredge et al., 2016). This is mainly in to raise the level of awareness among the society about the steps related to disease prevention. The Healthcare promotion program needs to be designed according to the requirement in each society. Essential data need to be gathered from the local population in order to identify the major diseases that are prevalent in the community. This will help me as a professional learn to design upon the health care program that are needed to promote. It is also essential to respect the cultural aspects in the community while implementing the Healthcare promotion program. This will help the professional nurse to better communicate with the people of the community and help them understand the importance of implementing preventive care. Essential data need to be gathered from the Pharmacology department in order to understand importance of each intervention diseases. The intervention strategies can be integrated with the practice of nursing that will help to manage the issues of chronic pain. The technology can also be integrated with the elements of the health care promotion. This will help the professional nurses to promote and generate awareness among the population.
Nursing Communication Plan
Population assessment is considered to be one of the important parts of nursing practice. This can help the Healthcare organisation of an area to identify the vulnerable communities. The major aim of the population assessment is to ensure that there is no form of discrimination that is being practiced while providing health care service. This is considered to be one of the essential ethical principles that are being followed.
As a professional nurse I need to identify the vulnerable group within the population, which is prone to more disease due to on healthy lifestyles. Unique style of Healthcare assessment needs to be implemented in those groups in order to ensure that they get the maximum benefit. It is effective to use certain Healthcare parameters in order to identify and diagnose the disease that are called within each individual. It will also be possible to identify the level of treatment that are required in each individual person.
Incorporating the ideas of Patient Safety is believed to be an essential part of clinical management that I have learnt from all the previous Encounters. The quality of Healthcare services that are provided to a patient highly depends upon the level of Patient Safety that is being maintained within the clinical setup (Hall et al., 2016). As the risk factor is minimised it is possible to provide advanced level of treatment to the patient depending upon their critical situation. The clinical management guidelines are believed to be an essential part of professional nursing practice. I do believe with effective implementation of clinical management it is possible for a professional nurse to overcome all type of Healthcare challenges. The effective patient safety can therefore be done with the help of the practices implemented within the clinical management. It can also be said that the quality of health service is highly dependent upon the levels of patient safety (Carayon et al., 2014).
I also do strongly believe upon the importance of evidence based practice. This has helped to develop upon my practical skills as a professional nurse. Evidence based nursing practice is mainly dependent upon accurate information that are gathered to patient care Technology. It is therefore possible to accurately diagnose the condition of a patient and identify the same from chronic and severe illness.
The health care policies that are implemented by the healthcare authority are highly dependent upon the needs and wellbeing issues within a population group. The health care financial plan will be implemented according to the specific needs of the population. With proper financial planning, it is possible to provide proper healthcare resource to the population.
Preparation for Emergency Situations
One of the major roles and scope of the health care policy is to understand the importance of providing high levels of social care. The health care policy needs to have detail information about the role of individual nurse. It is essential to have essential behavioural outcomes that are believed to have significant effects on to the cultural difference. There are essential elements of the interpersonal theories that has resulted into the identification and exploration of the different situations within the healthcare centre. The interpersonal theories have resulted in better levels of understanding of the patient situation. As a professional nurse, it is my duty to understand the psychology of individual nurse. This will help them to rely more upon the treatment procedures that are related with the resolution of the nurses. Hence, it is one of the professional duties of nurse to understand the psychological problems of the patients that can enhance the process of treatments (Dalgard et al., 2015).
The quality of my nursing skill is highly dependent on my communication skills. This will help me to maintain effectively maintain the nursing operational activity with all major interdisciplinary team within the healthcare system. This will help in the matters of dealing with the health care information that can be transferred easily within the departments. The quality of the healthcare service is dependent on the ability of nursing communication (Gausvik et al., 2015). It is also important to consider the fact that the basic concepts related to the health care promotion that has to be decided depending upon the needs of the certain groups of population.
I also need to highlight upon the theory of Trans cultural nursing, which is mainly aimed to deal with the issues of cultural difference within the community. By the application of this theory, it is possible to deal with the issues related cultural differences and discrimination that are faced by the community (Singleton, 2017). There is also the theory of system model that are needed to understand the environmental stressors. It is also possible to understand the client situation that will help to protect the nurses. This will help to ensure that most any nurse will be able to gather enough professional experiences in the context of the any given situation.
The holistic approach is mainly aimed to deal with the essential elements that can help in uplifting the mental strength of the patient. In other words, with the help of the holistic approach, it is possible to provide effective moral support to individual patient (Chen, 2015). With the help of the holistic approach, it is possible to deal with the issues of collaboration of the nursing approach within different forms of level. The information with the healthcare can be transferred from the primary health centre to the top most authority.
Promotion of Healthcare Services
The quality of healthcare service that is determined by the quality of nursing assessment can be judged by the overall capability of the nurse. The global dynamics is considered to crucial as it can help to gather essential ideas related to the nursing practice. This will help to deal with the different level of challenges that are encountered in each part of the world. The professional nurses need to understand the changes that occur within the healthcare sectors. This will help me as a professional nurse to effectively deal with all new challenges that are arise due to all type of critical factors.
Conclusion
I would like to conclude by highlighting upon few essential context related with that personal development plan for a professional nurse. Nursing communication and assessment tools are considered to be highly essential in the given context. This will help to ensure that as a professional nurse, I am able to implement effective decisions within a crisis situation. Respecting the cultural aspects every individual patient is also essential part of the ethical duty of every clinical nurse.
I would also like to highlight upon the nursing theories, which are effective in dealing with the interpersonal relationship between the healthcare workers and the patients. This will help to ensure that I as a trained nurse am able to provide the moral support that is needed for the treatment of critical patients. I have also decided to include the elements of holistic approach for enhancing the workplace environment of healthcare workers.
