Importance of Mental Health in Healthcare Delivery Systems
The healthcare delivery is evolving with the advancement of technologies and enhancement of knowledge. There has been continual improvement in the delivery of healthcare services that would have impact on the overall health conditions (Holland et al., 2021). It is essential to understand that both physical and mental health are important to improve the delivery of healthcare services and improving the health outcome among the patients or community. Mental health condition includes the social wellbeing and overall psychological and emotional element that would develop the significant affect on the thinking and feeling (Sutherland, 2020). The delivery of healthcare services is important along with considering the rightful strategic approaches and other essential quality that would help in preserving the key function within the settings. Therefore, to deliver the healthcare services, it is essential to consider and develop the performance measurement along with designing the patient care teams and care coordination that would have significant impact on the healthcare delivery system.
The paper will focus on certain practices that are commonly practiced within the mental healthcare services. However, such practices may develop risks of poor-quality care services and therefore, it is essential to discuss the key approaches and other consideration that would help in improving the overall condition within the settings. The paper intends to discuss the risks factors or poor outcome associated with the restrictive and defensive practice further discuss the alternative management strategies that would help in addressing the healthcare delivery.
The mental health services include the supporting care and significant treatment that would help patient to overcome the health condition. Mental illness has negative impact on the overall behaviours, mood and thinking capacity of the people. It is important to understand that mental illness have develop the addictive behaviours along with other anxiety disorders and depressions that would have significant impact on the overall health outcome (Samartzis & Talias, 2019). The provision of adequate mental healthcare services will have significant benefits to the quality of life and overall health outcome. Mental health services would improve the interpersonal skills, communication and self-esteem among the people. It is important to implement the capability to change the significant habits along with change in self-defeating behaviours that would help in improving health condition. There are two forms of intervention in providing the mental healthcare services: pharmacological and non-pharmacological intervention that would help in reducing as well as minimising the health issues within the settings (Druss & Goldman, 2018). It is important for both the approaches to provide the safety and addresses the key needs of the overall health conditions. Previously, it has been found that there is significant safety issue with certain practices within the mental healthcare services. The restrictive and defensive practices are identified as the key factor that contribute towards safety issues among the patients.
In the mental healthcare services, the incidents with violence and aggressions are common and it has also been found that there are potential possibilities of self-harm within the settings. Such violence and poor condition of behaviour have affected both service users or patients and healthcare providers. The restrictive practice is implemented to control such incidents however, it is considered as poor practice within the settings. The restrictive include restraint of the patients and it can be either done through manual or mechanical holding of the patients (Eales, 2020). It has been found that the seclusion is also the poor practice as it involves isolating the service users or patients in the locked room. The usage of sedating drugs (through injections) and continual observation are considered as restrictive practices. In many cases, it is considered as poor practice as cultural differences and poor impact on the overall understanding in improving the health outcome (Clark et al., 2017). It has also been found that restrictive practice within the mental health services is developing the traumatic experiences that would help in developing the significant outcome.
Risks and Poor Outcome of Restrictive and Defensive Practices
It has also been found that continual practice of restrictive care would result in physical harm and psychological harm that would have the significant damage and impact towards the overall development of therapeutic relationship. It is important that restrictive practice would also develop the significant violation of human rights. Strategy creators, scholastics, emotional well-being experts, customers and carers progressively perceive the need to lessen and, where conceivable, dispense with segregation and limitation (Clark, Hext & Xyrichis, 2018). Following are the different forms and types of restraint that has developed the negative impact on the overall condition within the mental health care settings.
Physical restraint: It is considered as the overall prevention or restriction for the movement of body. However, in many cases, it is implemented or practice without taking the essential consent. Although, it has certain usage in situation where, there is need for emergency to implement the practice of physical restraint. Thus, it is considered as the approach that would focus on preventing individuals from placing themselves into the dangerous situation that would have impact on harming on others.
Mechanical Restraint: In this form of restrain, there is usage of external device or equipment that would help preventing the movement or restrict the voluntary movement. For example, the deliveries of certain initiatives that would focus on improving the preventive measures among themselves. For instance, the usage of seat belt that would help falling out from the chair in the case of severe cerebral palsy. The usage of bed railing is another set of mechanical restraint that is provided to the people or service users.
Chemical Restraint: It is important to consider that include the usage of medication as well as substance that would impact on the medical practitioners. There is significant sole purpose to control and manage the behaviours of the service users. However, it is important to understand the consideration of medication prescribed under the healthcare professionals are not considered under the chemical restraints (Suliman, Aloush & Al?Awamreh, 2017).
It has been found that such defensive practices along with restrictive practice should be manage as it has been found that there are potential key challenges associated with the health condition. It may also include physical abuse and psychological abuse to the service users. It has been found that pinching, hitting, or pushing are the key examples of such physical abuse. In addition, it has also been understood the emotional manipulation and other threats that may contribute towards the psychological abuse. The practice of defensive approach such as application of chilli paste to the hand of service users to prevent any potential biting. Therefore, it is important to understand that such behavioural management practices are identified and considered as abusive as it have developed the significant impact on the overall delivery of healthcare services (Kinner et al., 2017). Such restrictive and defensive practices have also developed the ethical issues and ethical dilemmas that would have the impact on the safer environment along with improving the overall key outcome in improving the better delivery of quality outcome.
Pharmacological and Non-Pharmacological Intervention in Mental Healthcare
The implementation of restrictive practices reflects that there is lack of consideration towards the needs and rights of the people under any form of circumstances. Considering the duty of care and delivering the effective management of the healthcare services, it is important to develop the key preventive approaches. the exercise of self-control or separation should shape part of a Challenging Behaviour Intervention plan illustrating how, when, where and why the prohibitive practice is applied (Bland, Drake & Drayton, 2021). The arrangement should incorporate checking and audit arrangements guaranteeing the clients proceeded with security and prosperity. the Challenging Behaviour Intervention plan ought to be concocted through meeting with family, gatekeepers, direct help suppliers, administration directors, Disability Services’ Resource Team and some other fitting people. Wellbeing and safety should stay a significant part of emotional well-being nursing, genuinely supporting and enabling patients inside the clinic set chime includes suspending obtrusive and unsafe practices legitimized through the security talk as expressed and operationalized in dog lease nursing practice (Muir?Cochrane, O’Kane & Oster, 2018). To change the conceptualization and make dement of chance in mental long-term care, the idea of wellbeing itself should be rethought, and other consideration practices and systems focused on.
It is important to identify the associated issues and developing as well as identifying the management strategies that has developed the significant impact on the overall practice of health conditions. It is important to develop and identify the intervention that would help in implementing within the healthcare practices alternative to the restrictive practices. The behavioural change technique (BCT) taxonomy is considered as an effective approach they would help in addressing the key issues within the mental healthcare setting. The process is transparent as it would be developed based on the theory-based programmes that would include the roles of both patients and professionals. The BCT taxonomy would include the Delphi method, reliability of the coding and lastly the open-sort grouping tasks that would help in improving the computer program as well as consideration of effective statistical techniques (Baker et al., 2017). It would focus on the problem-solving approaches that will be based on the social needs and overall performance of behaviours. The role-playing will be considered that would be effective in developing the verbal de-escalation strategies that would help in improving the feedback and optimal emotional consequences within the behaviours and effectiveness.
The BCT implementation is considered as an effective approach by shaping the knowledge that would have consider towards the key element of shaping knowledge, followed by goals and planning. The interventions will be designed and developed based on the service users rather than healthcare staffs. Therefore, the intervention would impose the modification and other needful behaviours among the staffs within the settings. the use of information and implement the overall evidence-based approaches that would help in improving the overall environment in delivering the quality care. Thus, the BCT taxonomy is considered as an effective alternative solution that would help in addressing the needs of mental healthcare services among the service users (Berzins et al., 2020). The policy can be implicated or developed based on patient centric approach that would help in improving the overall delivery of health outcome.
Types of Restraint and their Impact on Overall Health Condition
Another significant intervention to develop the safety of the patients and enhance the overall delivery of health outcome among the mental health patients or service users. The shifting in the responsibility has developed the inpatient environment that would help in managing and addressing the burden associated with the responsibilities. It would also help in improving the anxiety as well as developing initiatives to address the burden. Organisational shifts were also considered that would help in developing safety within the inpatient environment (Slemon, Jenkins & Bungay, 2017). Therefore, as discussed, the verbal de-escalation is considered as the effective intervention that would help in risking the overall element for addressing the aggression. The intervention for usage is also consider as the key element that would have calm language, it would help in developing the communication techniques as well as help in diffusing the conflicting situation. It has been found that with the lower stimulus in the environment would also focus on reducing the potential noise or background noise that would help in lowering the overall adult mental health outcome is considered in providing the care with effective therapeutic relationship. It is generally found in the aged care settings as the level of background noise will be reduced and the same can be implemented within the nursing management practices.
Nursing professionals working in mental healthcare setting, it is important to implement and consideration of practice that would include the safety disclosures within the mature of care providers. The intervention is developed that would include the utilisation that would also hemp in managing the overall risks (Kang et al., 2020). Therefore, for providing the safety care within the clinical practices, it has been found that the nurses will implement the crucial components along with creating the significant environment that would have impact on the overall engagement and treatment.
Conclusion
From the above discussion, it can be concluded that the mental healthcare deliveries are complex and developed risks with the associated service. It has been found that there have been significant needs of services towards the restrictive and defensive practices within the settings. The paper has discussed the poor safety and other risks associated with the practice of such condition within the healthcare setting. therefore, the paper has successfully discussed the BCT approaches and other alternative strategies that would help in improving the overall health outcome.
